Nominators, please read this background information in full before submitting a nomination. Incomplete nominations will not be considered. Nominations that do not fit our criteria will also not be considered!!
BACKGROUND INFORMATION
Introduction
Impact on Education is pleased to continue our Impact Awards Program designed to honor five individuals in the Boulder Valley School District. The mission of the Impact Awards program is to recognize individuals in the district who have an extraordinary impact on student learning by valuing education, service to students, and leadership by example. We strive to identify the most inspiring individuals involved in public education in the District.
Individuals eligible for nomination include:
1. Certified classroom teachers and specialists;
2. Administrators such as principals, assistant principals, dean of students
3. District level administrators
4. Classified employees such as secretaries, custodians, para-educators, and bus drivers.
5. Volunteers
Each Impact Award recipient will 1) be honored at the Impact Awards Dinner, 2) be included in a video and media coverage, 3) receive 4 tickets to the dinner, 4) an award plaque and 5) receive a personal gift of a $500.00 check. Additionally the recipient's school will receive a check for $1,500.00 as a gift. Thank you for your interest in acknowledging excellence and submitting a nomination!

Impact Awards Dinner Thursday, April 17, 2008- CU Glenn Miller Ballroom
Criteria
The Impact Awards Program recognizes five individuals who meet the following criteria:
* Has a positive impact on students
* Inspires high personal and/or professional standards
* Motivates students to develop a life-long love of learning
* Provides or exhibits leadership
* Shares expertise and experiences with others
* Stands out from others in his/her field
Process
Do not mention the name of the nominee or the school in the nomination. Identifying nominee information is only allowed on the information form. Impact on Education uses a blind-read online process to evaluate applicants and select finalists. The readers will not know the name of the individual nominated or their school. This process requires that NO identifying information be included in the nomination narrative, such as names of individuals, buildings or specific programs. Use the individual's job title when describing him or her and refer to the school as "our school," or "our building." This is done to ensure impartiality and fairness in the selection process.
Ten nominees (more if there is a tie), who receive the highest median scores, will be selected as Impact Award Finalists. On Friday, February 1, 2008 each finalist will receive a phone call to congratulate them and to schedule a personal interview. All nominators will also be notified of the status of their nomination.
Letters of Recommendation (maximum -3) will be required in "hard copy" format for each Finalist. Please be prepared to submit the Letters of Recommendation by Friday, February 1, 2008 if your Nominee is chosen as a Finalist. The Letters of Recommendations may name the nominee and their school. The Interview Committee will review the Nominations and the Letters of Recommendations before the Finalists interviews. The Interview Committee will conduct a personal interview with each finalist to determine the five Impact Award Recipients. These interviews will be conducted February 6 and 7, 2008.
The Impact Awards Celebration will be held Thursday, April 17, 2008, at the Glenn Miller Ballroom on the CU campus, to honor the award recipients, finalists, and all Impact on Education grant recipients.
Note: Only one nomination for an individual will be considered. If more than one nomination is submitted for an individual, the nominators will be contacted and they will need to decide which nomination form shall be evaluated.
The 2007 - 2008 Impact Awards Information Form and Nomination must be submitted online to Impact on Education by 4 PM Friday, January 11, 2008.
We recommend that you copy and paste the application questions into Word and complete your application this way. By doing so, you can print it out and read it as one document and be sure you have a copy in case of an Internet connection failure, power outage, etc. When you have completed your application in Word, simply cut your responses and paste them back into your application fields.
